10. DOCUMENTATION
When a patient starts catheterisation, the following data must be collected and documented:
- reasons for catheterisation
- residual volume
- frequency
- date and time of catheterisation
- catheter type, tip, length and size
- problems negotiated during the procedure
Obtain verbal or written consent before starting the procedure.
Documentation should follow local policy.
